Wellness Program
The Colonial Group, Inc. Wellness Program is a free and voluntary wellness program available to all employees enrolled in the medical plan. The program is designed to promote overall health and wellness. Employees who choose to participate in the program in 2024 will pay a reduced premium for the 2025 Colonial Group Health Plan.
All Tier 2 and 3 participants qualify for the 2025 discount. Tier 1 will qualify if completed before December 17, 2024, you will receive the full discount and if completed after December 17, 2024, but before March 17, 2025, you will receive a discount from April - December.

Complete a Health Risk Assessment (HRA) that gathers information about your lifestyle and current or previous medical conditions (e.g., cancer, diabetes, or heart disease.
Have a biometric screening conducted, which will include a blood test for A1C, cholesterol, lipids, glucose, and other tests included in a Complete Blood Count (CBC).
2026 Medical Wellness Premium Discount: Similar to 2025, all employees will be eligible to earn the 2026 Medical Wellness Premium Discount. If you choose to participate in the wellness program, you will be asked to complete the following 3 activities January 1, 2025 to December 15, 2025:
Activity Requirement:
- Biometric Screening OR Annual Physical - PCP forms with biometric values will be submitted to SJ/C Clinic - Annual Physicals will be tracked by Healthgram
- Health Risk Assessment (HRA) - Complete the HRA on the Healthgram Portal, which can be accessed on the Dayforce HUB
- Meet with SJ/C Wellness Director to evaluate your overall health and establish personalized goals
If you are not able to meet the above activity requirements, you may meet with the Wellness Director six (6) times; or with your PCP to earn the wellness premium discounts.
